Who may be affected?

Everybody looks like a winner (the developing countries, the developed countries, the consumers, the producers). Even both sides of the ideological aisle could agree upon it, taking into account the status quo. Are there any losers with this proposal?

Well, of course, yes there are. Overall many would win, but the poor in the developed world would be excluded for longer from the enjoyment of certain technologies. There is an upside to this, though. Their governments are better equipped than many poor countries to help their own to offer access to essential technologies (in accordance with their view of social security). Regarding medicine, for instance, most developed countries have solid social security systems that could certainly handle it. Additionally, the fact that it is convenient for innovators would allow for more revenue to be collected in the developed world (discounting for tax avoidance), which in turn would increase budgets and could allow for the implementation of exactly those kinds of social security programs.
